There are many things in western countries that cost 20 to 40 times more than in India. Almost all professional and labor services fall in that category. The things that are cheaper or comparable in costs are mass produced items where labor and management compensation are diluted by the huge quantity produced by automation.

Some examples: (prices in US currency equivalents)

shoe polished in India $0.1-$0.2     USA $2.00 to $5.00

haircut India $ 1 typ. USA $15.00 typ.

Medical X-RAY and consultation $5.00 to $10.00 but may vary in India, USA @ $200 or so.

Plumbers and electricians charge $100 to $300 for the smallest job in USA, less than $5 in India.

Things like Appliances are cheaper in USA but now they are made in China.

Prescription medicine costs are very high in western countries due to a combination of factors. The cost of testing is high, Top management compensation is out of control in most companies, pharma and all. The patent system has an insidious effect on human behavior. Western societies believe that greed and profit are the only way to promote advancement in science and technology. They ignore the fact that the great geniuses of the past, Einstein, Maxwell, Tesla and thousands others were dedicated to advancement without the opportunities of financial enrichment. In fact the big rewards rarely go to the real geniuses in today’s corporations, the chemists, scientists etc.. They are mostly reserved for politicians without any science background. The head of NASA is probably paid less than the CEO of a sewer cleaning company! HP hires CEO’s with non-technical background like flower girls and politicians. Apple’s CEO came from a shoe marketing background!  Unfortunately, this greed based system is blindly followed by the rest of the world with its disastrous effects on the poor and the unfortunate.

Thats how inovation and competition works. !!  Reserving profit opportunities to only a few does not encourage innovation !
